• The project is physically complete but verification records have not been maintained or are non-existent;

• The installed equipment and systems are operational but an adhoc commissioning process was adopted and inadequate functional and performance tests were carried out;

• Project documentation deliverables such as as-built documentation, commissioning reports, relevant certifications, operations and maintenance manuals, spare parts, warranties, and training plans are not assembled to the satisfaction of the client; and

• Significant defects exist that the client demands must be rectified prior to practical completion being awarded.
